tongue over bit - please help

Please can someone give me some more information about why and how horses do this and what are the implications - especially regarding damage to the mouth.

Also, how do you know when a horse is doing this and how do you stop it happening?

I need to learn about this quickly, although the background is kind of complicated to explain.

Usually its because the bit is either set too low in the mouth or its too big.
